We're keen to find out what you think will make Cornwall and Isles of Scilly a healthier place to live and find out if you would be interested in working with us to shape future health and care services.
This survey asks some questions about what support could help you keep well and asks for your ideas that will help us shape a new health and well being prevention strategy and communications campaign.
We also want to recruit local people to join our engagement network so they can be kept informed and involved in shaping health and care services. The survey closes at the end of April 2024.
We will then be in touch with those who have asked to join our engagement network.

Do you, or someone you care for, have type 1 diabetes, type 2 diabetes, or gestational diabetes?
Or perhaps you have a family history of diabetes, or know someone well who has diabetes?
We are looking for local people across Cornwall and Isles of Scilly with lived experience of diabetes and diabetes services, to provide feedback and ideas to help us shape the way we provide diabetes care in the future.
What's involved?
We want to form a group of people with lived experience of diabetes, so we can ask for your help to provide feedback on current and shape future services.
If you join our group, there will be different ways for you to get involved and have your say, with both online / virtual and in-person opportunities available. This could include taking part in surveys, giving feedback in a group discussion, to being involved in work to 'design' future services alongside our staff.
You will be able to get involved in ways that suit you, and to share your feedback on matters that are most important to you.
Register your interest
Please complete this online form by Tuesday 21st May to register your interest. There is also a section on the form where we ask you what we can do to help make it possible for you to attend.
