
Shaping the continuing healthcare assessment process
NHS Continuing Healthcare (NHS CHC) is a package of care for adults aged 18 or over which is arranged and funded solely by the NHS. To receive NHS CHC funding, individuals must be assessed by integrated commissioning boards (ICBs) according to a legally prescribed decision-making process to determine whether the individual has a ‘primary health need’.
This process is set out in the The National Framework published by the Department of Health and Social Care.

This survey is about your experience of the services provided by Cornwall and Isles of Scilly Integrated Care Board's NHS CHC assessment and appeals process. We are seeking your views on how well the service works and how it can be improved.
This survey sometimes asks you to provide the reasons why you have given the answer you have, which you do not have to give. These questions are optional. If you do provide a reason, however, this will help us to improve the process for the future.
Please note that this survey is not a means of appealing or disputing the outcome of your assessment. There are separate processes for this, which your assessor can tell you about.

The survey results will be analysed regularly by the Continuing Healthcare Team, who are part of the NHS Cornwall and Isles of Scilly Integrated Care Board.
