This anonymous survey is created to understand your views on the Covid-19 Vaccine. Our aim is to understand what might drive you to get it, but also any concerns you may have. Please be as detailed as possible. Your opinions will help inform our messaging around the vaccination programme aimed at under 25's.

Once the survey has closed, your data will be held within Cornwall Council’s secure network and premises for up to 2 years. Access to your information will only be made to authorised members of staff who are required to process it for the purposes outlined in this privacy notice.

The survey results will be shared with the NHS as part of the planning for a joint Covid-19 vaccination campaign. This survey does not hold any health data and we will not be sharing any personal information.
