We are creating new services in Cornwall to help adults (18+) stay well, live independently, and have a home. This includes support with mental health and trauma, finding and keeping a home, and day to day living skills. Your feedback will directly shape these services.
• All questions are optional.
• You can answer for yourself or for someone you support (as a carer/family member).If you are a carer or family member, please answer from the perspective of the person you support where possible.
• Estimated time: 15 minutes.
