The report will cover the period 1st April 2020 to 31st March 2021 and will close at noon on Friday 12th November 2021.
The report will include information about:
Summary of what your organisation does and who you work with.
The contract(s) you deliver for Together for Families.
The number of children, young people and families you have worked with in delivery your contract.
A summary of the difference you have made for children, young people and families in Cornwall.
Your key performance outcomes for the year.
What children, young people and families say about the service(s) you deliver on our behalf.
What social and added value you have brought into Cornwall.
Your risks and opportunities and also your innovation.
We have designed this survey to collect the information from you which will form the basis of the Together for Families contract performance report. Please note your completed survey will be appended to the report and will be made available as a public document.
We know all our contracted providers are committed and work incredibly hard for children, young people and families in Cornwall. You are dedicated to supporting and improving the outcomes for children and we want to highlight the excellent services and outcomes that you deliver, as well as highlighting the opportunities and challenges. Your data will be stored securely and will only be accessible by members of Together for Families. Your consent for your data to be used in the evaluation will be assumed from you starting the survey.
Survey responses will be collected using the Let’s Talk Cornwall survey software. By completing this survey, you consent to this transfer of your information. This includes any information which may represent your personal data. All the information you share with Together for Families will be stored safely for the duration of the evaluation. It will be destroyed one year after it has finished.
