Once the survey has closed, your data will be held within Cornwall Council’s secure network and premises for up to 2 years. Where we ask you to supply personal data on this form, we will only make access to this data available to authorised members of staff who are required to process it for the purposes outlined in this privacy notice. Please note that other information you provide, such as responses to open questions, may be published in full.

The survey results will be shared with the Cornwall and Isles of Scilly Integrated Care Board, NHS England, the Cornwall and Isles of Scilly Pharmaceutical Committee and any partners represented on the Cornwall and Isles of Scilly Health and Wellbeing Board.

According to legislation, a PNA must contain:

  • A statement of the pharmaceutical services provided that are necessary to meet needs in the area;
  • A statement which outlines any gaps in essential pharmaceutical service provision in the area, where services are needed and are not currently provided (gaps in provision);
  • A statement of the other non-essential pharmaceutical services provided in the area;
  • A statement of the services that are not being provided, but which would, if they were to be provided, improve or make it easier to access pharmaceutical services in the area;
  • A statement of other NHS services provided by a local authority or the NHS, which affect the needs for pharmaceutical services;
  • An explanation of how the assessment has been carried out (including how the consultation was carried out);
