This feedback tool has been created to support the improvement of the Early Help system, including defining what success looks like. The data will be informed by the voices of families, practitioners and managers.

Please only complete the survey that you were asked to complete in the email you received.

There are two parts to each question. The first asks you to assign a score based on the scoring criteria below. For the second part we are asking if you could kindly provide examples and evidence to support your score.

The scoring criteria are presented below for your reference.

0. There are no or few elements of this descriptor in place with no plan for development.

1. Planning has started and is at an early stage of development. It is too early for evidence of impact.

2. There is a plan to achieve this and some evidence that this is being implemented. It’s too early to demonstrate impact/outcomes from this work.

3. There is some good evidence of progress – to some extent / across many elements. There may be some emerging evidence of the outcomes/impact. The next steps are clear.

4. This is largely in place although not yet fully established or embedded. There is some good evidence of outcomes/ impact. There is a plan for continuous development.

5. This is in place and well established. There is strong evidence that developments are having impact where needed. There is a commitment to continuous development.

Your data will be stored securely and will only be accessible by members of Care and Wellbeing. Your consent for your data to be used in the evaluation will be assumed from you starting the survey.

The evaluation is being carried out by Care and Wellbeing. Please see our general privacy notice.

How will you store my personal data?

Survey responses will be collected using the Let’s Talk Cornwall survey software. By completing this survey, you consent to this transfer of your information. This includes any information which may represent your personal data. All the information you share with Care and Wellbeing will be stored safely for the duration of the evaluation. It will be destroyed one year after it has finished.

Let’s Talk Cornwall is the system used to collate the data and create reports from it. You can view the privacy statement on the Let’s Talk Cornwall website.
