Creative, Health and Wellbeing Partnership Survey
The Creative, Health and Wellbeing Partnership is moving forward with a model of inclusivity, engagement and distributed leadership, enabling all members to play a role in how it develops. It is very important to us to hear what you would like from the Partnership, why are you interested and what are your expectations for getting involved.
We have designed a short, 5-minute survey in order to build the work of the Partnership around our members.
The survey will be open until the end of Wednesday 30th June and Jayne from Arts Well will circulate results to you shortly afterwards.

The Data Controllers for the information you provide in this survey is Jayne Howard, Arts Well South West and The Culture and Creative Partnerships team at Cornwall Council. Once the survey has closed, your data will be held within Arts Well's and Cornwall Council’s secure network and premises for up to 2 years. Access to your information will only be made to authorised members of staff who are required to process it for the purposes outlined in this privacy notice.
