Safe and Well Hub Review

‘All my care in one place’

The aim of the Cornwall Safe and Well Hubs was to test a new approach. We want to improve the experience and outcomes for people experiencing:

• homelessness
• mental and physical ill-health
• domestic abuse and sexual violence
• offending behaviour,
• drug and alcohol problems

Often, people experience two or more of these things together. This means they have to go from service to service and appointment to appointment. It can take a long time to get help with everything. Often things got worse in the meantime. Also, a lot of these services didn't always work together well.

The important elements of the model are:

• different services working together in one place
• improved accessibility (you can drop in without making an appointment)
• flexible support, centred on the needs of residents
• Peer-led initiatives

The hubs are flexible spaces that include:

Shared desks for professionals (hot-desking)

• Bookable meeting, interview and clinical rooms
• Kitchen and communal facilities

We currently have Safe and Well Hubs in 4 locations:

• John Daniels Centre, Penzance
• Polkyth House, St Austell
• Pydar House, Truro
• South Wheal Crofty, Redruth
