  • Please complete our short survey to give your views on living with HIV in Cornwall and the Isles of Scilly. This survey will close on 01/05/2024.

    You are being invited to complete this online questionnaire for people living with HIV in Cornwall and the Isles of Scilly.  This is being collected by Jo Canneaux in the Public Health team in Cornwall Council.  

    The questionnaire is anonymous and aims to understand more about how it feels to live with HIV in Cornwall and the Isles of Scilly.  The information gained from this questionnaire will be shared with people who work and commission HIV services to further understand the local needs of people living with HIV and to improve local services.   

    The questionnaire should take 15 minutes to complete online.  Many questions are multiple choice, and there are some answers with free text where we would encourage you to tell us your personal experiences.  

    Completing this questionnaire is voluntary and you can stop at any time.  You do not have to answer all the questions.  We cannot remove individual responses once the questionnaire has been submitted because we will not know which responses came from you

    We will use the information you provide to inform the HIV Joint Strategic Needs Assessment.

    The Data Controller for the information you provide in this survey is Cornwall Council, New County Hall, Treyew Road, Truro TR1 3AY. Data Protection Registration Number: Z1745294. Once the survey has closed, your data will be held within Cornwall Council’s secure network (Wellbeing and Public Health system) and premises for up to 2 years. Where we ask you to supply personal data on this form, we will only make access to this data available to authorised members of staff who are required to process it for the purposes outlined in this privacy notice. Please note that other information you provide, such as responses to open questions, may be published in full.

    The anonymous survey results will be published as part of the HIV Joint Strategic Needs Assessment which will be available later in 2024 here: Reports and papers - Cornwall Council
