Every year we run an engagement programme to gather feedback from local people on their experiences of health and care services and ask for ideas on how we can shape the future of health and care services.

You can answer some or all the questions depending on your interest and time. This survey is designed in a way that allows you to skip sections you don’t want to complete.

We will share your feedback with health and care service leads and use the findings to shape future services and plans.
