School health checks are carried out on an 'opt out' basis. If you do not wish your child to have them, you need to 'opt out' by completing the form below.
If you are happy for your child to be weighed and measured at school there is no need to contact us.
Please note: Opt outs may take up to 5 working days to process.
If weighing and measuring is due to take place within the next 5 working days, please notify the school office.
