Survey for Cornwall Day opportunities and inclusion
This survey is for;
1: Individuals who currently use day services
2: Families or carers of someone using day services
3: Individuals who may consider using day services in the future.
Cornwall Council aims to create services that enable people to live the life they desire. Cornwall Council is working with National Development Team for inclusion (NDTi) to speak to a range of people across Cornwall about their experiences of day opportunities and inclusion to understand what individuals expect from day opportunities now and in the future.
The National Development Team for inclusion (NDTi) is an organisation that aims to make things better for people who need support. For more information, please visit our website: www.ndti.org.uk
We're interested in hearing from:
- Individuals who currently use day services.
- Families or carers of someone using day services.
- Individuals who may consider using day services in the future.
We want to know what matters most to you. To ensure everyone can share their thoughts, we're:
- Conducting a survey.
- Visiting day services and talking to people online.
- Collaborating with local people who use day services as part of our team.
After gathering insights, NDTi will compile a report for Cornwall Council and other interested parties. This information will assist Cornwall Council in planning future day opportunities across Cornwall enabling service user choice.
This survey has 10 questions, and your participation is optional. It's also completely anonymous, so your identity and participation will remain confidential. Please note that we cannot remove your answers if you change your mind, because we won’t know who said what.
Is there anything we can do or put in place which would make it easier for us to offer you an equal service? For example documents in large print, hearing loop, interpreter. Please use the space below or email adultscommissioning@cornwall.gov.uk
