We would like your help in evaluating Bloom Consultation Meetings.

The survey asks a number of questions about your experience of attending these meetings and will help us in thinking about how we might improve them.

All information is anonymised, and participation in the evaluation is entirely voluntary. You do not have to give your name, job title or organisation but you do have to complete the date of the meeting you attended, in which locality you attended and whether you were present as a nominated professional or ‘other’ attendee.
